Problems with video_fullscreen_scale = 3 (integer scale)

It's hard to try and run through things and test. So many combinations of stuff.

I started having some crash problems and narrowed it down to issues with the integer scale mode. I'm using a Surface Book with Performance Base.

I get a blank screen with this combination

video_fullscreen_scale = 3
gfxcard = vga

video_fullscreen_scale = 3
gfxcard = et4000ax

These cause crashes if you start with the integer scale. If you start in no scale and run the computer, then switch to integer, you get a blank screen.

video_fullscreen_scale = 3
gfxcard = px_trio64

video_fullscreen_scale = 3
gfxcard = n9_9fx

video_fullscreen_scale = 3
gfxcard = bahamas64

(I did not try all combinations)
