What drivers are you using? What machine? I don't encounter this issue, even with built-in drivers.

IMO I also don't think a configurator program would be useful. That's overblowing featurecreep and going against KISS principles that make PCem already elegant as it is. Virtualizers that have them need them because they're designed for enterprise users in mind, which is more for a 'everything must...

Just tested with DOSBox-x, also happens there, with the built-in s3mm.drv as well as the ID (CRTC 0x30) set to 0xe1.

Still happening, ffs. And yes, this is definitely a bug, since it doesn't happen in VPC2007 or on DOSBox and they ought to emulate it normally (this is on s3mm.drv, not s3_2.drv or s3.drv).

No crash here either.
Edit: the Voodoo1 might be the reason why it crashes, switch to Voodoo2 instead.

Much like the 2011 (kinda), you need an already bootable DOS HDD to configure the machine's internals like floppy boot, etc.

I recently added the RAMDAC with the correct values as well as the vclk of it, 15bpp and 16bpp work in 640x480 and 800x600 modes, but 1024x768 mode seems to not render correctly (8bpp mode is unaffected of the same resolution). 160KB and 320KB are handlled in PCem v11, it's probably the PCjr itself that can't handle the formats, but I don't know.

Actually, the S3 Trio64 was introduced the same time as NT 3.5 and as the Mach64, and the Vision864 doesn't allow 1024x768x32bpp.

After setting the monitor settings (in Advanced Settings), Default Monitor, change it to Custom, then do the Monitor resolution tests.
Once this is done, it will ask you about to reprogram the nvram, select yes and exist and reboot win95.

Run it on DOS or Windows 95 DOS mode (don't even rename the file with another name) and then follow what the on-screen instructions say.

Mach64GX acceleration memory limit
Is it really limited to 4mb? Because, for a bare test, Win95 glitches a lot with 8mb, yet with acceleration turned off completely it renders correctly.
Note: this does not apply to NT (any version above 3.1 since no mach64 drivers exist for such version).
