In this game, while running it in either a Windows 9x DOS window or real mode MS-DOS, the screen appears to be duplicated at the top, and it flashes rapidly.
This issue occurs on the S3 Virge 325 and DX, the ATI Mach64 cards, and the PCI Trident cards. (these are the cards that I tested for this issue)
I have attached screenshots of this issue shown on the S3 Virge 325.
In addition, I have tested for this issue in the latest version of DosBox, and it does not happen there.

Looks broken. Tried it on the Diamond Stealth 32 and it flickers like heck.

The game's readme file mentions that it has issues with Tseng-based graphics cards (which is most likely real behaviour), and for a change in the in the configuration file to prevent it.
I will test it on the S3 Trio-based graphics cards to see if they show the same issue as well.
(Edit: The screen duplicating issue occurs on the S3 Trio64, Trio32, Virge DX and Vision864 cards, the Cirrus Logic PCI graphics cards, the ATI Mach64 GX and the Mach64 VT2, but not the flickering. The flickering and screen duplicating issues occur on the S3 Virge 325, Tseng ET4000/w32p PCI, and the Trident TGUI9440).

I've think i've seen this buffer flicker on real hardware in Windows 98 before on a real S3 Trio64V+* card a very long time ago (~'99). This is kind of why they made a Windows version (Incredible Toon Machine) not long after. Whether if it involved S3VBE/SDD/UNIVBE I can't remember...
Probably even weirder is that I had originally played this game with a Tseng ET4000 in '94 and there wasn't a problem
